Welcome to Greenagers. We're looking forward to working with you!

If you will be under 18 at the time that you start the program - please fill out this form with your parent/guardian. You will both need to sign and provide various information.

Before you get started:

The onboarding forms include:

1. Registration Form
2. Medical/Emergency Contact Form
3. Photo Release Form
4. Optional and Anonymous Demographics Survey.

You will need the following information in order to complete these forms:

• Your date of hire and department (Trails, Farm, Trades/Build, Education or Office)
• Emergency contact information, and your allergies or other medical conditions.
• Upon completion of these forms, you will be provided with a separate link to the W4 and I-9 forms: for these, you will need your Social Security Number and your tax withholding information.

The forms can be saved and resumed later.

Participant Agreement

By signing below, I acknowledge that I will be expected to work hard and to maintain a good and positive work ethic throughout the season. I understand that the work is physical by nature; that it requires me to work as a contributing member of a team; and that I will be expected to preserve professional behavior while at the workplace and in transit.

I certify that all information provided here is accurate to the best of my knowledge.
